Role Summary:
- Responsible for delivering a comprehensive array of physical therapy services, which includes assessment, treatment planning, and therapeutic interventions within a collaborative environment, adhering to professional qualifications and ethical standards. Demonstrate accountability for contributions to program development, quality enhancement, problem-solving, and productivity improvement in a multidisciplinary setting.
Qualifications:
- A minimum of a Bachelor’s degree from an accredited program (or equivalent). Current licensure in the state of practice is mandatory. Current BLS Certification is required.
Experience Required:
- Background in assisting patients with rehabilitation, catering to individuals with disabilities or injuries across their lifespan.
Key Responsibilities:
1. Exhibit and maintain a professional demeanor for patients, families, colleagues, and other stakeholders.
2. Conduct initial evaluations, select appropriate assessment tools, and determine suitable adaptive equipment/materials based on the patient's age and developmental stage. Ensure that patient assessments are thoroughly documented in the medical record as per established policies.
3. Formulate personalized treatment plans with measurable short- and long-term objectives, specifically addressing functional deficits. Interventions are to be chosen based on assessment findings and tailored to the patient's age, cognitive abilities, and developmental level, which may include administering prescribed medications.
4. Assess patient responses to treatment. Adjust treatment plans as necessary based on patient progress towards goals. Provide updates to patients and families regarding current status, program planning, and discharge processes throughout the rehabilitation journey.
5. Document patient care progress meticulously to substantiate the necessity for skilled intervention versus lack of progress, and communicate further clinical recommendations to referring providers. Ensure daily documentation aligns with policy and procedure, including progress notes and discharge summaries.
6. Educate patients and families on tasks and resources that promote ongoing functional independence. Involve family members in treatment, education, and discharge planning to facilitate a safe transition home and ensure continuity of care. Evaluate the effectiveness of education through peer feedback, family input, and patient satisfaction metrics.
7. Engage in continuous professional development and education activities. Participate in departmental training sessions and attend external professional growth opportunities, sharing insights with the team.
8. Comply with all relevant federal and state regulations, professional standards, and organizational policies. Familiarize oneself with and adhere to the contents of applicable professional codes and organizational handbooks.
9. Achieve established productivity standards while maintaining fiscal responsibility.
