Job Summary

We are seeking a highly skilled Habilitation Specialist to join our team. As a Habilitation Specialist, you will be responsible for providing direct care services to individuals with developmental disabilities, overseeing the implementation of Staff Action Plans, and ensuring compliance with billing requirements.

Key Responsibilities
  • Provide direct care services to individuals with developmental disabilities
  • Oversee the implementation of Staff Action Plans and services
  • Ensure compliance with billing requirements and associated documentation
  • Provide clinical input and documentation at Life Plan meetings
  • Transport individuals in multiple-sized vehicles
Requirements

To be successful in this role, you must have a Bachelor's degree in a human services field and be able to drive from one location to another. You must also be able to transport individuals in multiple-sized vehicles.
