Key Responsibilities- Conduct thorough psychosocial assessments to identify patients' needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure seamless transitions and discharge planning.
- Provide crisis intervention and support to patients and families in need.
- Develop and maintain knowledge of community resources and social programs to support patients' needs.
- Communicate effectively with patients, families, and healthcare teams to ensure high-quality care.
- Masters Social Work Degree required.
- LGSW/LCSW/LICSW certification in the state of West Virginia required.
- One to three years of experience preferred.
