Position Overview:

SSM Health is seeking a dedicated Registered Nurse to provide exceptional care in the home health and hospice setting. This role is pivotal in delivering quality healthcare services to patients in their homes, ensuring their comfort and well-being.

Key Responsibilities:

  • Conduct thorough assessments of patients' health conditions and coordinate care plans with the healthcare team.
  • Perform essential nursing tasks including vital sign monitoring, wound care, IV therapy, and patient education.
  • Collaborate with physicians to update and modify care plans based on patient progress and needs.
  • Document all patient interactions and care provided, ensuring compliance with healthcare regulations.
  • Engage with patients and their families to develop personalized care strategies that address their unique needs.
  • Participate in discharge planning and ensure smooth transitions for patients moving out of care.
  • Maintain a safe and alert working environment while performing nursing duties.

Qualifications:

  • Graduate from an accredited nursing program.
  • Valid Registered Nurse license in the state of practice.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • No prior experience required; new graduates are encouraged to apply.

Physical Requirements:

  • Ability to communicate effectively with patients and healthcare team members.
  • Frequent standing, walking, and lifting of patients or equipment.
  • Proficient in using technology for documentation and patient care.

Work Schedule:

This position requires a commitment of 32 hours per week, primarily during daytime hours, with some on-call responsibilities.
