Key Responsibilities:

  • Assist patients with personal care tasks as outlined in their care plan, including hygiene, mobility, and other essential activities.
  • Ensure a safe living environment for patients by using equipment correctly and safely.
  • Adhere to Universal Infection Control Precautions during all patient interactions.
  • Support patients and families with household tasks as specified in the care plan, such as light cleaning and meal preparation.
  • Monitor and document patients' fluid and nutrition intake, vital signs, and any changes in their condition.
  • Implement safe transfer and ambulation techniques, ensuring patient comfort and safety.
  • Respect patient confidentiality and autonomy at all times.
  • Maintain accurate documentation of care provided and patient responses, submitting records promptly.
  • Operate within the scope of your training and experience.
  • Comply with all relevant laws, regulations, and agency policies.
  • Manage a caseload effectively while delivering high-quality care.
  • Consistently meet or exceed established service standards.
  • Conduct all professional activities ethically and with integrity.
  • Utilize supplies and equipment efficiently.
  • Complete a minimum of 12 hours of continuing education annually.
  • Maintain regular attendance and punctuality.
  • Adhere to the organization's Code of Conduct.
  • Engage positively with all staff, supporting the mission and vision of 365 Health Services.
  • Follow agency policies regarding confidentiality and HIPAA guidelines.

Job Types: Full-time, Part-time, Temporary

Salary: $13.00 per hour
