Job Summary:

Mission Healthcare is seeking a skilled Physical Therapist to join our team in Ventura County. As a leading healthcare provider, we deliver high-quality, patient-focused care in the comfort of our patients' homes.

Key Responsibilities:

  • Provides physical therapy services to patients according to a written physician's plan of care.
  • Evaluates and assesses therapeutic, rehabilitative, and functional status, and participates in the development of the total plan of care.
  • Directs physical therapy treatment, instructs patients and families/caregivers in the use and care of therapeutic appliances, and determines priority needs for physical therapy.
  • Reports to the physician on patient's reaction to treatment or changes in condition, and initiates physical therapy programs.
  • Supervises homemaker/home health aides when participating in patient's rehabilitation regime, identifies patient and family/caregiver needs for other home health services, and refers as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals, participates in discharge planning for patients, and provides physical therapy consultation to home families/caregivers when indicated.

Requirements:

  • Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the State.
  • Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
  • Demonstrates good verbal and written communication and organization skills, and possesses and maintains current BLS Hands On CPR Certification.
  • Travel is required for this position, and must maintain a valid driver's license, active auto insurance meeting state minimum requirements, and an operational vehicle.
