Cornerstone Rehab is seeking a skilled Occupational Therapist to join our team. As a clinician-owned and operated company, we prioritize patient outcomes and deliver innovative rehab management solutions to skilled nursing facilities across the United States.

**Key Responsibilities:**

• Provide occupational therapy services to adult and geriatric patients

• Develop and implement individualized plans of care to meet patients' needs and capabilities

• Prepare and maintain accurate documentation, progress notes, and discharge summaries

**Requirements:**

• Valid State Occupational Therapist License and Credentials

• Experience in SNF and/or Acute care settings

• Covid Vaccine required in certain locations
