About the Role

Nuvance Health is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients achieve independence in their daily lives.

Key Responsibilities
  • Develop and implement individualized plans of care in partnership with patients, representatives, and caregivers.
  • Provide services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.
  • Communicate with physicians and other healthcare practitioners to ensure seamless care coordination.
  • Maintain accurate records of patient visits, including evaluations, plans, and client responses.
  • Participate in case management conferences to coordinate occupational therapy programs with total care plans.
  • Provide education and counseling to patients and family members on occupational therapy treatment regimens.
  • Identify patient and family needs for other home health services and make referrals as needed.
Requirements
  • Current valid driver's license and access to a reliable vehicle.
  • Bachelor's degree in Occupational Therapy.
  • CT State Occupational Therapy License.
  • Minimum one year of experience.
