Avera Medical Group Family Medicine/Avera Now Mitchell is seeking a skilled Clinical Social Worker to join their team. This role offers an exciting opportunity to provide essential social work services to patients and families within a clinical setting.

Job Summary:

The Clinical Social Worker will be responsible for addressing medically related emotional and social needs of patients through assessment, planning, and referral to community resources. This position requires a Master's Degree in social work from a program accredited by the Council on Social Work Education, as well as 1-2 years' healthcare social work experience.

Key Responsibilities:
  • Provide direct clinical therapeutic services to patients as part of an overall comprehensive patient care plan
  • Maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals
  • Assess needs of patients facing chronic illness and facilitate plans with patients and families to promote effective care
Requirements:
  • Certified Social Worker (CSW-PIP) License in the State of South Dakota required
  • Quality Mental Health Professional Credential (South Dakota) preferred
  • Master's Degree in social work from a program accredited by the Council on Social Work Education
  • 1-2 years' healthcare social work experience
Compensation:

The hourly range for this position is $28.75 - $43.50 per hour, with actual hourly rate dependent upon experience. The company offers competitive wages with regular increases and benefits.
