Job Summary:

The Clinical Pharmacist Specialist will be responsible for ensuring safe, appropriate, and cost-effective medication therapies are made available for patients according to established policies, procedures, and protocols.

Key Responsibilities:
  • Develop and implement treatment guidelines, protocols, formulary changes, and critical pathways.
  • Identify, resolve, and communicate cost-effective medication use issues in collaboration with the medical staff.
  • Provide clinical consultation and clarification to practitioners and patients, suggesting appropriate and cost-effective therapeutic alternatives as needed.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions, and appropriateness of drug/dose.
  • Prepare and present drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports, and pharmacy intervention summaries to the Pharmacy and Therapeutics Committee.
Requirements:
  • Doctorate of Pharmacy (PharmD)
  • PGY1/PGY2 Oncology residency required
  • Licensed Pharmacist in the state of VA
  • Board certified Oncology Pharmacist or certification obtained within two (2) years of employment
