About the Role:
The Registered Nurse Case Manager will be responsible for planning, organizing, and directing home care services, utilizing their expertise in nursing and community health education. This role requires a strong emphasis on building relationships with patients, families, and caregivers to ensure seamless care coordination and exceptional patient outcomes.
Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of patients and their families
- Collaborate with healthcare professionals, community resources, and family members to ensure comprehensive care
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
- Provide education and support to patients and families on health management, disease prevention, and wellness
- Coordinate and manage home care services, including medication management, wound care, and other specialized services
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and timely interventions
Requirements:
- Current and valid Utah RN licensure
- One to two years of recent acute care experience in an institutional setting
- Basic Life Support (BLS); hands-on CPR certification
- Valid driver's license and auto insurance
Preferred Qualifications:
- Bachelor's degree with one to three years of home health care experience
Compensation and Benefits:
Mission Healthcare offers a competitive hourly rate of $30.00 to $36.00, as well as a comprehensive benefits package, including 401(k) and opportunities for professional growth and development.
