APN/PA - Outpatient Care Manager

Job Summary:
Loyola University Health System is seeking an Advanced Practice Nurse (APN) or Physician Assistant (PA) to join our team as an Orthopedic Specialist. This is a full-time position that will provide advanced care in the outpatient setting.
Key Responsibilities:
- Manage patients with orthopedic conditions in an outpatient setting, functioning independently and collaboratively with physicians, case managers, and other healthcare team members.
- Facilitate patient transitions between service units and provide complex patient and family teaching.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Nursing – Current Registered Nurse License - State of Illinois
- Nursing – Nursing Practitioner License – State of Illinois
- Specialty certification in a clinical or functional area of nursing granted by a national nursing organization is preferred
Preferred Qualifications:
- EPIC system experience
- Ability to become proficient in EPIC with on-the-job training
