Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Community Options, Inc. in River Edge, NJ. As a LPN, you will deliver high-quality care to individuals in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
Key Responsibilities- Provide treatment and healthcare procedures ordered by a physician and required by the standards of professional practice or law
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
- Advise supervisor of all concerns and issues related to the provision of nursing services
- Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
- Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
- Train employees on the medical needs of assigned individuals
- Monitor health and medical conditions of individuals and refer for medical services as needed
- Monitor the use of medications through medication administration logs
- Ensure health service supplies are available and replenished as needed
- Ensure program documentation and billable records are completed accurately and timely
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
- Participate with individuals in community, social, and recreational activities
- Participate in circles of support for personal futures planning
- Follow strict OSHA procedures regarding blood borne pathogens
- Additional tasks and responsibilities may be assigned
- Bachelor's degree or equivalent experience in nursing field
- Minimum of one year of experience providing nursing services to individuals with disabilities
- Valid nursing license
- Valid driver's license with a satisfactory driving record
- Complete all state and agency required training per state guidelines
- Ability to calculate figures related to medication dosages
- Understanding and commitment to community-based support for persons with disabilities
- Ability to exercise good judgment and remain calm in crisis situations
- Ability to establish a comfortable and supportive relationship with an individual receiving supports
- Proficient with Microsoft Office
- Excellent verbal and written communication skills
- Ability to effectively present facts and recommendations
- Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community
