Job Title: Full-Time Residential Case Manager at Clitheroe Center

We are seeking a highly skilled and compassionate Full-Time Residential Case Manager to join our team at Clitheroe Center. As a key member of our staff, you will provide direct case management services to our clients, helping them to achieve their goals and improve their overall well-being.

Job Summary:

The Residential Case Manager will work closely with clients to identify their needs and develop personalized plans to address them. This will involve assisting clients with housing and employment searches, making necessary appointments, and connecting them with other resources and services. The ideal candidate will have a strong understanding of case management principles and practices, as well as excellent communication and interpersonal skills.

Responsibilities:
  • Provide direct case management services to clients, including housing and employment searches, appointment making, and resource connection.
  • Develop and implement individualized plans to address client needs and goals.
  • Work closely with clients to identify and address barriers to success.
  • Collaborate with other agencies and organizations to ensure availability of services for clients.
  • Document client services and progress in electronic health records system.
Requirements:
  • Bachelor's degree in field related to social services preferred.
  • One year of experience in substance abuse, homelessness, or related field required, or five years' experience can be substituted for a bachelor's degree.
  • Skilled in case management, discharge planning, housing and job search activities, and working with persons with mental health and substance use disorders.
  • Knowledge of cross-cultural issues and DBH requirements.
  • Understanding of trauma-informed care.
  • Alaska driver's license with acceptable criminal history and driving records.
Work Environment:

The majority of work is performed indoors with a quiet-to-moderate noise level. The Residential Case Manager will be required to sit, stand, walk, climb, push, pull, stoop, kneel, bend, and reach on an intermittent or continuous basis. Hearing and vision must be correctable to within normal ranges. Ability to operate general office equipment and drive a motor vehicle in darkness or inclement weather conditions is also required.
