About the Role:- Provide skilled nursing care to patients in their own homes, utilizing the nursing process to plan, implement, and evaluate patient care.
- Initiate and revise patient care plans as needed, in collaboration with physicians and other healthcare professionals.
- Administer medications and treatments as prescribed, and monitor patient response to interventions.
- Review patient care plans and identify areas for healthcare teaching, care coordination, goal setting, and discharge planning.
