We are seeking a skilled Registered Nurse to join our team at Abode Hospice and Home Health. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to our patients and their families.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Administer medication and treatment as prescribed by the physician, and provide education to patients and families on proper medication management.
- Counsel patients and families on meeting their nursing and related needs, and provide emotional support and guidance as needed.
- Document patient progress and maintain accurate clinical notes to ensure continuity of care.
- Communicate effectively with the physician and participate in interdisciplinary group meetings to ensure optimal patient outcomes.
- Ensure compliance with all state and federal legal and regulatory requirements, and adhere to our company's policies and procedures.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least 1 year of nursing experience, with 3+ years preferred.
- Experience in home health or a similar setting is highly desirable.
- Commitment to clinical and documentation excellence, with a focus on delivering high-quality patient care.
