Advanced Practice Provider

National Recruiters is seeking an Advanced Practice Provider to join our team in conducting health risk assessments on Medicare Advantage members. As an Advanced Practice Provider, you will have the opportunity to work independently, assessing the health risks of individuals and providing recommendations for improvement.
Your primary responsibilities will include conducting in-home health risk assessments, identifying areas of concern, and developing plans to address these issues. You will also educate patients on healthy living habits, disease prevention, and management strategies. Additionally, you will document patient interactions and maintain accurate records using our digital platform.
Compensation: $80-$120 per hour, with incentives for high-volume providers. A minimum of five or more 7.5-hour days per month is required, with opportunities for additional hours based on demand. Travel may be necessary to nearby counties. A valid NP/PA license in any specialty (excluding women's health and pediatrics) is essential for this position.
