Job Description
As an LPN with CenterWell Home Health, your primary responsibilities will include:
- Providing skilled nursing interventions to patients with complex conditions, such as chronic wounds, diabetes, or heart disease.
- Developing and implementing individualized care plans, incorporating evidence-based practices and patient preferences.
- Collaborating with multidisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
- Maintaining accurate and timely documentation, adhering to regulatory requirements and company standards.
- Participating in ongoing education and training to enhance knowledge and skills, staying current with industry advancements and best practices.
Requirements
- Associate degree or diploma in practical nursing from an accredited institution.
- Current LPN licensure in the practicing state.
- Valid driver's license, vehicle insurance, and reliable transportation.
- Ability to travel extensively throughout the assigned area, visiting multiple patients daily.
- Two years of experience as an LPN in a clinical setting, preferably home health or hospice.
