Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at New Vista of the Bluegrass, Inc. The successful candidate will be responsible for providing case management services to individuals and families in need of support and guidance.

Key Responsibilities
  • Provide direct face-to-face services to link clients to resources and programs to address identified needs
  • Assist clients in developing a Person-Centered Recovery plan of care
  • Provide ongoing monitoring to ensure services are being furnished in accordance with the plan of care
  • Provide advocacy services on behalf of clients and their families
  • Collaborate and communicate with clients, families, and community partners to ensure seamless service delivery
Requirements
  • Bachelor's degree in psychology, social work, or other behavioral science related field
  • One year of full-time employment experience working directly with adults/children in a human service setting
  • Valid driver's license, reliable operating transportation, and proof of current auto insurance
Preferred Qualifications
  • Master's degree in social work or related field
  • Knowledge of mental health issues, substance use issues, and community supports
  • Knowledge of Person-Centered Recovery based interventions in Case Management services
