Job Overview Summary:

We are seeking a skilled Licensed Practical Nurse to join our team at Plymouth Harborside Healthcare. As a key member of our nursing staff, you will provide high-quality patient care while working collaboratively with our interdisciplinary team to achieve positive clinical outcomes and patient satisfaction.

Key Responsibilities:
  • Contribute to nursing assessments and care planning, providing direct patient care and supervising non-licensed staff.
  • Collect, report, and document objective and subjective data, observing conditions and reporting changes to RNs and nursing management.
  • Implement the plan of care, evaluating effectiveness and making adjustments as needed.
  • Administer medications and perform treatments per physician orders, communicating pertinent data to RNs and physicians.
  • Participate in shift-to-shift communication, collaborating with other departments to provide timely and effective care.
Requirements:
  • Graduate of an accredited school for practical or vocational nursing.
  • Valid LPN license in the state employed.
  • Valid CPR certification.
  • Ability to read, write, speak, and understand the spoken English language.
