Job SummaryWe are seeking a skilled Speech Therapist to join our Home Health team. As a Speech Therapist, you will be responsible for providing high-quality therapy services to our patients, including evaluations, assessments, and treatments. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct comprehensive evaluations and assessments to identify patient needs and develop individualized care plans
- Provide high-quality speech therapy services, including individual and group sessions, to patients in their homes
- Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each patient
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and training to stay current with best practices and industry developments
- Master's degree in Speech-Language Pathology from an ASHA-accredited program
- Valid professional SLP license in the state of practice
- Experience working with patients in a home health setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
