Job Description
We are seeking a motivated Physical Therapist to join our practice full-time in Prospect, CT. Our state-of-the-art, interdisciplinary offices are looking for a PT to provide coverage at our locations in Middlebury and Prospect, approximately 15 minutes apart. We collaborate with patients and other healthcare professionals to provide comprehensive care.

Duties and Responsibilities
• Patient Consultation: review health and medical histories, examine patients, and review pertinent medical records/imaging
• Consult with referring Physicians/Healthcare team regarding diagnosis as needed
• Develop and implement physical therapy treatment programs based on each patient's needs
• Assist patients with all phases of physical therapy treatment and techniques, including exercise instruction and activities of daily living
• Patient education on the benefits of PT, treatment plan maintenance, and home exercises
• Documentation/progress notes: accurate and timely records, document treatments rendered in patient charts, and record patient conditions and responses to treatments in charts after each visit
• Collaboration and coordination of care with other healthcare professionals for comprehensive patient care
• Communicate effectively with both the staff and patients

Requirements
• Graduation from an Accredited Physical Therapy Program
• PT License in CT
• Strong Orthopedic background preferred
