Health Home Care Manager Position

Open Door Family Medical Center seeks a highly skilled Care Management Specialist to join its team in Purchase, NY. This role will play a crucial part in improving health outcomes for adults with Medicaid who have chronic medical and behavioral health conditions.
The ideal candidate will possess excellent communication skills, be fluent in English and Spanish, and have a strong background in healthcare and/or social services. With a minimum of two years' experience, they will be able to work effectively in a fast-paced environment, prioritize tasks, and collaborate with internal and external service providers.
About the Role- Conduct assessments to identify individualized needs related to psychosocial well-being, physical health, and social determinants of health.
- Collaborate with enrollees to set goals and develop a care plan, monitoring goal achievement and revising the care plan as needed.
- Coordinate and arrange for medical, behavioral health, social, and other relevant services, including appointment scheduling, reminders, and transportation booking.
