- Provide one-on-one therapy sessions to patients in their homes, focusing on improving mobility, strength, and overall function.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes, using data to inform treatment decisions and improve patient outcomes.
- Current licensure as a Physical Therapist in the state of operation.
- Minimum nine months of experience as a Physical Therapist.
- Preferred experience in community or home health settings.
