About This Role

We are seeking a friendly and compassionate Travel Nurse Practitioner to join our team, supporting patients into the Patient Health Management (PHM) program. In this role, you will work closely with care coordinators across various providers and settings to promote improved health outcomes.

Key Responsibilities

  • Assist care coordinators across different providers and settings to ensure seamless communication and coordination of patient care.
  • Promote better health outcomes by facilitating effective communication between facilities and patients, and ensuring accurate documentation through our proprietary software system.
  • Cross-train in other areas of care management to expand your skill set and contribute to the success of our team.
  • Gather and analyze patient information, review pertinent labs, studies, and provider notes, and assess patient condition to inform care decisions.

Requirements

  • Nurse Practitioner License and DEA License
  • At least 6 months experience in long-term care
  • Reliable transportation for traveling to different nursing homes within Massachusetts

Compensation Package

We offer a competitive salary range of $150,000 - $200,000 per year, along with a comprehensive benefits package, including:
