Job Summary

Walden Family Services is seeking a highly motivated and experienced Education and Employment Specialist/Case Manager to join our team. The successful candidate will be responsible for preparing young adults to enter the job market and maintain stable employment. This role requires travel between Encino & Riverside office locations.

Key Responsibilities
  • Performs job-coaching duties for participants assigned to their caseload, serving as a positive role model and modeling appropriate work skills, behavior, and social interaction.
  • Utilizes various job coaching techniques to meet participant and employer needs, including identification and analysis of work to be performed, aiding the participant in mastering job duties, and ensuring that work performed by the participant meets employer expectations.
  • Provides follow-along interventions to participants assigned to caseload, including counseling, advocacy, job coaching, skill training, assessment, and referral.
  • Collaborates with a team approach to share information on participant progress or other relevant information, including Supported Employment members, participants, Supported Employment Coordinators, family, advocates, and other support members.
  • Participates in developing and reviewing bi-annual Supported Employment Work Plans for participants assigned to their caseload.
  • Participates in training to improve skills in the use of structured training techniques for teaching job performance skills, job-related skills, and work behaviors.
  • Schedules training, virtual, and in-person, and completes JBAY stats.
  • Provides supervision of program participants at work sites, not to exceed 8 program participants per work site.
  • Responsible for documentation of services provided, case notes, notable occurrences, processing of time sheets, and attendance records to ensure compliance with federal, state, and agency regulations.
  • Stays knowledgeable of transitional housing programs and resources for transitional-aged former foster youth.
  • Conducts pre-placement appraisals with placing agencies and youth, explaining program expectations and completing necessary intake paperwork.
  • Completes initial and update needs and services plans/STLP and submits to the supervisor within 10 working days.
  • Provides at least weekly consultation to the youth, discussing progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, housing advocacy, and tenancy training, computer training, transportation assistance, and assisting in contact with appropriate supportive adults/mentors.
  • Assists with managing the youth's cash resources, personal property, and valuables.
  • Assists with maintaining accurate records of the youth's bank account, savings, and monthly budget.
  • Provides and/or coordinates referrals for additional services, consultants, and community resources as needed (i.e., medical, therapy, psychiatric, parenting classes, job training).
  • Documents the youth's weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts.
  • Completes special incident reports and any other reports as required by the placing agency.
  • Maintains contact with placement workers and informs them of any significant events/incidents.
  • Consults with the supervisor regarding incidents, significant issues, and possible terminations, and follows up as directed.
  • Meets with the supervisor for a minimum of one hour weekly of individual supervision and case consultation.
  • Ensures that client files are complete and up-to-date.
  • Submits all reports and documentation to the supervisor for final approval.
  • Facilitates trainings and support groups as directed by the supervisor.
  • Projects a favorable image of Walden Family Services at all times.
  • Performs other duties as assigned or requested.
