Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team at Providence Home and Community Care (HCC). As a Registered Nurse Home Care Specialist, you will provide high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Associate's or Bachelor's degree in Nursing.
  • Minimum 3 years of experience in acute care or community behavioral health settings.
  • Geriatric experience preferred.
