- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with physicians and other healthcare team members to ensure comprehensive care.
- Administer medications as prescribed and document patient responses.
- Provide education to patients and their families on disease management, medication use, and other health-related topics.
- Monitor patient progress and adjust care plans as needed.
- Current RN license and valid driver's license.
- At least one year of experience as a Registered Nurse, with a minimum of six months in Home Health care.
- Current BLS certification.
- Valid auto insurance.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
