Key Responsibilities
  • Manage a caseload of home care clients, including scheduling, coordinating care services, and conducting reassessments as needed
  • Communicate with clients and their families to establish care plans and ensure their needs are being met
  • Recruit, interview, hire, and educate a team of home care providers to deliver exceptional care services
  • Monitor and ensure compliance with all state regulations and company policies
  • Coordinate with healthcare professionals and other community resources to provide holistic care for clients
  • Maintain accurate and timely documentation of all client care activities
Requirements
  • Bachelor's degree in healthcare management or a related field preferred
  • Minimum of 2 years of experience in home care management or a similar role
  • Knowledge of state regulations and policies related to home care services
  • Excellent communication and interpersonal skills
  • Ability to multitask and prioritize tasks effectively
  • Proficiency in computer skills, including electronic health record systems
  • Must be able to work independently and remotely with minimal supervision
