Aveanna Healthcare, a leading provider of pediatric and adult home health care, is seeking a compassionate Licensed Vocational Nurse (LVN) to join our team. As a skilled nursing care specialist, you will provide high-quality care to patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Job Summary

The Licensed Vocational Nurse (LVN) will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians' orders. This role requires a strong attention to detail, effective problem-solving skills, and excellent communication abilities.

Key Responsibilities
  • Deliver and document skilled nursing care in accordance with the care plan and physicians' orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
  • Participate in the development and implementation of the nursing care plan.
  • Provide education to patients and families regarding disease processes, self-care techniques, and prevention strategies.
  • Maintain accurate and timely documentation of patient care and progress.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One (1) year of experience required working under current nursing license.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
