POSITION SUMMARY: Aperion Care Lakeshore is seeking a dedicated Registered Nurse responsible for delivering direct nursing care to residents while overseeing the daily nursing operations conducted by nursing assistants. This role requires adherence to federal, state, and local regulations, ensuring the highest standards of quality care are consistently met.

KEY RESPONSIBILITIES:

  • Supervise the daily activities of nursing assistants. Collaborate with nursing staff and support personnel to plan services, programs, and activities for the shift. Document and communicate reports and recommendations regarding shift activities as necessary. Manage the admission, transfer, and discharge processes for residents, providing leadership to the nursing team assigned to your unit.
  • Complete and maintain required documentation for resident admissions, transfers, and discharges. Accurately receive and transcribe physician orders, ensuring they are recorded appropriately. Maintain thorough and descriptive nurse's notes that reflect the care provided and the resident's responses. Prepare and submit incident reports to the Director as needed. Adhere to established charting and documentation policies.
  • Administer medications as prescribed by physicians, ensuring compliance with medical directives.
  • Foster effective communication and collaboration with inter-departmental personnel and other facility departments to ensure nursing services meet resident needs. Cultivate a warm, personal, and positive atmosphere while maintaining a calm environment throughout the unit. Participate in physician rounds as required.
  • Provide professional nursing services, including catheterization, tube feeding, suctioning, dressing changes, and other necessary procedures.
  • Monitor nursing staff to ensure compliance with safety regulations regarding equipment and supplies.
  • Maintain confidentiality of all resident care information, including protected health information. Report any known or suspected breaches of confidentiality. Ensure that nursing care is delivered in a private manner, with staff knocking before entering resident rooms.
  • Provide care for IVs and IV sites as necessary.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Current and unencumbered state licensure as a Registered Nurse.
  • Proficient in reading, writing, speaking, and understanding the English language.
  • Able to make independent decisions when circumstances require.
  • Knowledgeable about nursing and medical practices, as well as relevant laws and regulations governing nursing care facilities.
  • Current CPR certification is mandatory.
  • Experience in Long-Term Care is preferred.

PHYSICAL REQUIREMENTS:

  • Ability to move throughout the workday.
  • Capable of managing the mental and emotional demands of the position.
  • Must meet general health requirements as outlined by facility policies, including a medical and physical examination.
  • Able to lift, push, pull, and move a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.
  • May need to assist in evacuating residents during emergencies.
