The Sub-Acute Care Nurse serves as a clinical role model and offers nursing consultation to colleagues within their designated areas of responsibility.

About the STEP Program

The STEP (Skills, Transitions, Exploration, and Progress) program is a collaborative initiative between Kaiser Permanente and Seneca, designed to deliver Partial Hospitalization services to youth. The primary objective of the STEP program is to provide intensive, short-term stabilization services, enabling clients to transition to lower levels of outpatient mental health treatment. The program emphasizes teaching essential skills for mental health awareness, psychoeducation, and personal safety. A multidisciplinary team, including counselors, clinicians, health information specialists, nurses, psychiatrists, and program supervisors, supports the STEP program.

Key Responsibilities

  • Assess and evaluate patient conditions.
  • Monitor and manage physician orders for medications and treatments.
  • Foster collaborative relationships with Pediatricians, Specialists, Psychiatrists, Therapists, and hospitals.
  • Coordinate appointments and treatment orders requiring nursing expertise.
  • Provide ongoing training to staff regarding medication management.
  • Deliver health-related education to both staff and clients.
  • Document all nursing interventions, including medication assistance records.
  • Collect and analyze assessment data to identify treatment issues and develop individualized care plans.
  • Review medical incident reports for the assigned shifts.
  • Facilitate recreational activities for the program.
  • Support the therapeutic environment as necessary.
  • Manage and facilitate the referral and intake processes.
  • Assist with client transportation when required.
  • Participate in multidisciplinary team meetings.
  • Collaborate with the Program Director and Psychiatrists as needed.
  • Work with the Psychiatrist to ensure quality medication administration and follow-up care.
  • Partner with the Clinical Manager to uphold health and safety standards, infection control, and clinical care protocols.
  • Attend treatment reviews related to youth healthcare.
  • Provide on-call support for nursing inquiries as necessary.
  • Employ crisis communication and de-escalation techniques as per Seneca's training, which may include physical management of clients when applicable.

Qualifications

Required

  • Bachelor's degree in Nursing.
  • Active Registered Nurse license in the relevant state.
  • Current CPR-BLS certification.
  • Minimum age of 21 years.
  • Clearance from TB tests, fingerprinting, and other state/federal licensing requirements.
  • Valid driver's license with a clean driving record.
  • Proof of up-to-date COVID-19 vaccination.

Preferred

  • Experience in Psychiatric Nursing is preferred.

Work Schedule

  • Monday to Friday, 8:00 AM to 4:00 PM.
