Job Summary

We are seeking a skilled Patient Advocate Representative to join our team at Conifer Revenue Cycle Solutions. As a Patient Advocate Representative, you will play a critical role in ensuring that patients receive the financial assistance they need to access quality healthcare.

Key Responsibilities
  • Conduct thorough interviews with patients to determine their eligibility for various government and non-government programs.
  • Identify and pursue all potential sources of payment, including auto insurance, Workers' Compensation, commercial insurance, and private insurance.
  • Obtain and complete confidential financial statements, and assist patients in applying for benefits.
  • Develop and maintain strong relationships with patients, hospital staff, and community resources to ensure seamless care coordination.
  • Travel to patient homes for skip tracing and document collection, as needed.
Requirements
  • High school diploma or equivalent required; minimum 2 years of experience in social services, hospital admitting, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Working knowledge of federal, state, and county programs, as well as proficiency in Microsoft Office and Windows.
  • Bi-lingual skills (Spanish) a plus.
Work Environment

This role requires the ability to work in a fast-paced hospital environment, with frequent travel to patient homes and community resources. The ideal candidate will be able to sit for extended periods, walk across campus settings, and visit patients at bedside.
