Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work in a variety of settings, including post-acute care, and provide high-quality patient care.

Key Responsibilities
  • Deliver post-acute care to patients, including monitoring vital signs and administering medications
  • Conduct intakes and document patient health concerns and medical histories
  • Review patient medical records and charts
  • Administer medications and verify their accuracy
  • Maintain a safe and comfortable environment for patients
  • Assist with IV set up, vaccinations, and phlebotomies
  • Follow and adhere to care plans for patients
  • Assist with medical staff orientation and training
Requirements
  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience as an LPN or in a similar role
  • Ability to lift a minimum of 50 lbs
