We are seeking a skilled Physical Therapist to join our team at Northwestern Memorial Healthcare. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our Outpatient Rehabilitation Services department.
Key Responsibilities- Develop and implement individualized physical therapy treatment plans to maximize patient function and independence.
- Evaluate patients and gather relevant information to determine accurate clinical diagnoses and treatment plans.
- Communicate treatment plans to patients, families, and interdisciplinary teams, providing emotional support as needed.
- Coordinate and implement treatment plans, modifying as necessary based on patient progression.
- Treat patients using a variety of techniques, including therapeutic exercise, functional activities, manual techniques, and modalities.
- Evaluate home, work, and community settings and make recommendations for safe and efficient function.
- Re-evaluate patients at appropriate timeframes and document goal progression to maintain effective plans of care.
- Plan and communicate discharge needs to patients, families, and medical personnel, reviewing and finalizing exercise programs and practices for independent and safe care.
- Bachelor of Science in Physical Therapy
- Current State of Illinois Physical Therapy license or in process of obtaining
- BLS certification through the American Heart Association
