Key Responsibilities- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and implementing interventions as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Practical Nursing Degree from an accredited school.
- Current LPN license in our state or ability to obtain within 90 days.
- Basic Life Support certification through the American Heart Association.
- IV Certification within 1 year.
