Our multidisciplinary care teams collaborate with patients, their families, and other healthcare providers to deliver personalized, evidence-based care tailored to each patient's unique needs.

We serve patients residing in Assisted Living, Memory Care, and Group Home communities throughout Minnesota, Wisconsin, and Florida.

In addition to primary care, Bluestone has a highly developed care coordination model for more than 14,000 seniors and individuals living with disabilities in Minnesota.

Care Coordinator Role Overview

As a Geriatric Care Manager, you will work with the senior population managing Department of Human Services (DHS) and Center for Medicare/Medicaid Services (CMS) required activities for Minnesota Senior Health Options (MSHO/MSC+) members living in the community and assisted living facilities.

Responsibilities:
  1. Coordinate face-to-face visits with patients
  2. Manage the Elderly Waiver program
  3. Conduct annual assessments, including PCA assessments, for community members and customized living tools for members residing in Assisted Livings
  4. Review current health needs, identify goals, and develop individualized care plans
  5. Connect patients with community and state resources and services
  6. Complete required documentation
  7. Collaborate with medical care teams to ensure quality measures are met and utilize management tools to meet value-based goals
  8. Support patients during transitions of care and collaborate with their care team to ensure a safe discharge and follow-up plan

The estimated salary for this position is $70,000 - $77,000 per year, based on experience.

Job Requirements
  • Current Minnesota Registered Nurse license or MN Licensed Social Worker
  • One or more years of experience working with the geriatric population in case management/care coordination, Home Care, Nursing Home, TCU, or Assisted Living settings preferred
