Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Wound Care Coordinator - RN. As a key member of our nursing center, you will play a vital role in leading and integrating a patient-centered approach to skin health and wound care.

Key Responsibilities
  • Lead and develop a comprehensive Skin Health and Wound management program in collaboration with nursing center management.
  • Provide clinical expertise in skin, wound, and incontinent products, supporting central supply and durable medical equipment related to wound prevention and management.
  • Collaborate with the interprofessional team to ensure adherence to national wound guidelines and standards of care.
  • Develop and implement education programs for staff on wound care and skin health best practices.
  • Conduct regular audits and assessments to ensure compliance with regulatory requirements and nursing home standards.
Requirements
  • Current RN license (MPT/DPT with leadership review/approval)
  • Completion of Skin Health Team Lead orientation training and Wound Basics within 90 days of hire
  • Commitment to attend monthly support calls
  • National accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or commitment to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire
  • Strong knowledge base of clinical charting, incident reporting, and investigation response
  • Experience in team building and demonstrating respect for the interprofessional team in wound management
  • Knowledge of national wound guidelines and wound products
  • Knowledge of nursing home regulations and survey process
  • Strong Microsoft Word, Excel, Database Management, PowerPoint skills
