Overview

The Home Health Registered Nurse (RN) plays a crucial role in delivering high-quality patient care within the comfort of their homes. With proficiency in Oasis for patient care management, they coordinate and supervise LPNs and HHAs to ensure agency compliance with Medicare and Medicaid regulations.

Key Responsibilities

  • Conduct initial home visits, reassess patient needs, and monitor progress regularly.
  • Develop and implement personalized care plans under physician's orders, ensuring seamless coordination with other healthcare professionals.
  • Evaluate and document patient symptoms, reactions, and progress, making informed decisions to optimize treatment outcomes.
  • Empower patients and caregivers through education on disease processes, medications, and treatment plans, promoting self-management and autonomy.
  • Coordinate patient services, supervise LPNs and HHAs, and notify physicians of changes in patient condition, maintaining open communication channels.
  • Discharge patients when discharge criteria are met, ensuring a smooth transition to the next stage of care.
  • Foster collaboration with clinicians to ensure coordinated care, leveraging expertise to enhance patient outcomes.
  • Update clinical records and maintain knowledge through ongoing education, staying abreast of best practices and regulatory requirements.

Qualifications and Requirements

    • Graduate of an approved nursing program.
    • Current licensure in the practicing state.
    • Minimum one year of RN experience, with a strong foundation in case management and patient advocacy.
    • Exceptional communication and interpersonal skills, with the ability to build rapport with diverse stakeholders.
