Job Summary

We are seeking a highly motivated and detail-oriented Support Broker to join our team at Public Partnerships LLC. As a Support Broker, you will work with Medicaid members who participate in self-direction programs, providing education and support to help them manage their care and services.

Key Responsibilities
  • Visit participants in their homes to provide information and education on self-direction and care management
  • Evaluate home environments and identify safety concerns, working to resolve them through our mandated escalations process
  • Assist participants in creating and managing their care plans, ensuring they have the support and resources they need
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
  • Utilize Microsoft Office and CRM tools to manage caseloads and document interactions
Requirements
  • BA or BS degree in social work, case management, or related field
  • 1 year experience working with vulnerable populations in their homes
  • Strong working knowledge of Microsoft Office and CRM tools
  • Familiarity with electronic medical records and self-direction principles
  • Valid driver's license and reliable automobile
