We are seeking a skilled Advanced Practice Clinician to join our team at Optum Home & Community Care. As a vital member of our Senior Community Care product, you will play a key role in providing high-quality care to patients in their homes, nursing homes, and assisted living facilities.

Salary

The estimated hourly range for this role is $42.31 to $83.27 per hour, with opportunities for professional growth and development. Our comprehensive benefits package includes competitive pay, incentives, recognition programs, equity stock purchase, and 401k contribution (all subject to eligibility requirements).

Job Description

As an Advanced Practice Clinician, you will be responsible for delivering medical care services to assigned members in a periodic or intermittent basis. Your expertise will encompass:

  • Primary care delivery, including comprehensive assessments and documentation of findings
  • Care coordination, focusing on ancillary services, plan development, and implementation
  • Program enhancement, promoting positive customer relationships and adapting to changing circumstances
Required Skills and Qualifications

To succeed in this role, you must possess:

  • A Certified Nurse Practitioner degree through a national board
  • Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
  • Active and unrestricted license in the state where you reside
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Access to reliable transportation for travel to client and/or patient sites
  • Ability to lift a 30-pound bag in and out of car and navigate stairs and various dwelling conditions
