Job Title: Physical Therapist

At Sigma Healthcare, we are seeking an experienced Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The Physical Therapist will be responsible for initiating physical therapy programs, instructing other personnel and/or family/caregivers in certain phases of physical therapy, and participating in case conferences. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work effectively in a team environment.

Responsibilities:
  • Provides physical therapy services to patients according to a written physician's plan of care.
  • Assesses and evaluates therapeutic/rehabilitative/functional status and participates in the development of the total plan of care.
  • Directs physical therapy treatment and instructs patients/caregivers in the use and care of therapeutic appliances.
  • Determines priority needs for physical therapy and reports to the physician patient's reaction to treatment or changes in condition.
  • May train patients in the use of prosthetic devices and identifies patient and family/caregiver needs for other home health services/hospice and refers as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals and participates in discharge planning for patients.
Requirements:
  • BLS/BCLS certifications
  • Valid Driver's License
  • Valid state license (where applicable)
  • SNF experience - Required
