The Registered Nurse (RN) Case Manager plays a vital role in coordinating client care and serves as the primary contact for all healthcare disciplines involved in patient care. Key responsibilities include overseeing visit frequency, consulting with physicians regarding patient care details, performing accurate OASIS data collection, maintaining up-to-date medication profiles, ensuring timely reporting of lab values to physicians, and participating in weekly case conferences and monthly meetings.
QualificationsTo qualify for this position, Registered Nurses (RNs) must meet the following criteria:
- Hold a current Registered Nurse (RN) license in the state of employment
- Possess a minimum of one year of clinical experience, preferred
- Exhibit knowledge and proficiency in contemporary nursing practices
- Have a valid state driver's license and automobile liability insurance
