About the Position

This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Coordinates care plans to ensure optimal patient outcomes
  • Collaborates with interdisciplinary teams to develop and implement care plans
  • Identifies potential barriers to care and develops strategies to overcome them
  • Provides education and support to patients and families
  • Maintains accurate and up-to-date records
Requirements

To succeed in this role, you should have a minimum of five years' experience in a similar position, preferably in an acute care or community health care setting.

Preferred Qualifications:

  • Bachelor's degree in Nursing (BSN) or higher
  • Advanced certification in Case Management or a related field
Compensation and Benefits

The estimated annual salary for this position is between $90,000 and $120,000, depending on experience.
