Care Management Specialist

As a Service Coordinator I, you will play a critical role in supporting individuals in achieving their person-centered goals and living as independently as possible. You will assess, identify, secure, and monitor services for program participants based on their unique needs, and respond to participant needs through various communication methods and remain attentive to their requirements.
Key Accountabilities:
• Assess and develop individualized service plans that meet the unique needs of program participants.
• Identify and secure necessary services and resources to support program participants.
• Monitor and evaluate the effectiveness of services and make adjustments as needed.
• Collaborate with internal and external stakeholders to ensure seamless delivery of services.
Requirements:
• Education: Bachelor's degree REQUIRED (in social work, psychology, or related fields preferred). Practicum experience is a plus.
• OR Experience: At least three years in a social service or healthcare-related setting REQUIRED. Experience with people with disabilities or seniors and/or at least one (1) year of LTSS, Service Coordination, or Case Management experience preferred
Salary: $21-$24 per hour.
