Job Title: Case Manager MSW

At Great Plains Health, we are seeking a highly skilled Case Manager MSW to join our team. As a Case Manager MSW, you will play a vital role in providing comprehensive care coordination for patients, ensuring they receive the best possible care and support throughout their journey.

Key Responsibilities:
  • Assess patients' bio-psychosocial needs and develop personalized plans of care, taking into account age-specific needs.
  • Collaborate with the Cancer Center team to provide collaborative care and planning for patient needs related to their diagnosis, treatment, and decision-making.
  • Perform daily screenings and participate in inter-departmental team meetings to identify patients in need of intervention.
  • Document patient information and provide continuity of care with other team members.
  • Provide crisis intervention and support groups to patients and their families.
Requirements:
  • Master's degree in Social Work required.
  • Current certification by the State of Nebraska in Social Work (CSW) required.
  • Current Nebraska driver's license required.
Physical Demands:
  • Stand and/or walk frequently.
  • Sit frequently.
  • Bend, stoop, and crouch occasionally.
  • Reach floor to overhead occasionally.
  • Lift and/or carry 25 pounds 100 feet occasionally.
  • Push and/or pull 25 pounds occasionally.
