At The Moments, we are seeking a compassionate and skilled Licensed Practical Nurse to join our team. As a key member of our nursing staff, you will play a vital role in providing high-quality care to our residents.

Key Responsibilities:

  • Ensure the health and safety of all residents are met.
  • Lead and direct your care team, actively performing cares of each resident as needed.
  • Collaborate with residents, families, physicians, and Moments staff to create a person-centered plan of care that enhances the life and care of the resident.
  • Communicate effectively with residents, family members, physicians, all vendors, and RN Supervisors.
  • Prioritize and organize work effectively and efficiently.
  • Commit to a compassionate care philosophy and whole person approach.
  • Excellent judgment and decision-making abilities and problem-solving skills.
  • Experience with EHR/EMR (electronic health/medical records), ideally Point Click Care.
  • Support the orientation and education requirements of staff.
  • Ability to work with a team to develop person-directed care and outstanding sensory and cognitive programming, in a culture of compassion, respect, and dignity.

Requirements:

  • Must be licensed with the State of Minnesota as an LPN and must maintain current and unencumbered licensure.
  • Must complete a criminal background check from Department of Human Services and may not be on the OIG exclusions list.
  • Must be screened for TB.

Job Types: Full-time, Part-time

Salary: From $30.00 per hour experience based *shift differential $.50 evenings $1.00 NOC shift
