Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team in Shiloh, Illinois.

About the Role

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist will develop a treatment plan, perform interventions to improve patient well-being, and evaluate progress.

Responsibilities
  • Provide occupational therapy services within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with other healthcare team members.
  • Evaluate patient functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consult with the physician in the development of the therapy plan of care.
  • Observe, record, and report to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress towards goals.
Requirements
  • Current Occupational Therapy licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation
