* Collaborate with multidisciplinary teams to design, develop, and implement individualized program plans that align with the goals and preferences of clients.
* Monitor program effectiveness and adjust as needed to optimize outcomes.
* Ensure CARF compliance and stay abreast of accreditation requirements.
* Conduct regular self-assessments and internal audits to identify areas for improvement.
* Provide training and ongoing support to program staff, ensuring they have the necessary knowledge and skills to meet CARF standards and deliver high-quality services.
* Establish and maintain a system for continuous quality improvement, including data collection, analysis, and reporting.
* Advocate for the rights and needs of clients, ensuring they receive the highest standard of care and support.
* Maintain accurate and up-to-date records in compliance with CARF standards and regulatory requirements.
* Generate comprehensive reports for internal and external stakeholders as needed.
* Foster positive relationships with community partners, local agencies, and other stakeholders to enhance resources and opportunities for clients.

Requirements

* High School Diploma or equivalent.
* Minimum of 3 years of experience in program coordination or related role within the IDD field.
* Ability to lift 35 pounds and perform physical tasks associated with the position.
* Non-standard, flexible schedule based on program needs.
* Travel as required for training, program, community, and company events.
