Job Title: Family Medicine Physician

We are seeking a highly skilled Family Medicine Physician to join our team at Penn Medicine Lancaster General Health. As a Family Medicine Physician, you will be responsible for providing high-quality patient care in a unique practice setting that focuses on chronic medical conditions.

Job Summary:

The successful candidate will have the ability to work in a fast-paced environment, providing ambulatory, home, and inpatient care to patients. You will be part of a team-based care approach, working closely with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other healthcare providers to deliver comprehensive care.

Key Responsibilities:
  • Provide ambulatory, home, and inpatient care to patients with chronic medical conditions
  • Attend to patients enrolled in the program in the hospital on a daily basis in a consultative fashion
  • Take after-hours call in rotation with other medical staff to provide 24/7 coverage for patients via phone
  • Participate in team-based care with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other healthcare providers
  • Consult with patients to determine the appropriate course of treatment
  • Recommend and order tests to determine the extent of illness or help diagnose conditions
  • Monitor lab results, medication adherence, and medication toxicities
  • Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury
  • Explain and discuss procedures, test results, or prescribed treatment with patients
  • Refer patients to medical specialists when necessary
Requirements:
  • Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
  • Completion of Internal Medicine Residency or Family Medicine Residency
  • Board Eligible or Board Certified in Internal/Family Medicine
  • Current medical licensure issued by PA State Board of Medicine or the PA State Board of Osteopathic Medicine
  • A healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs
